Do I need to apply for FMLA medical leave?

  • Having a medical procedure…in-patient or out-patient. YES!
  • Need to take care of an injured family member. YES!
  • Have a chronic condition that I need time off periodically to treat. YES!
  • Need maternity or parental leave. YES!

Paid Family Medical Leave (PFML)

WA State’s paid leave program is administrated outside of WWU by the Employment Security Department. To find out more about this program at https://paidleave.wa.gov

Apply for Medical Leave in 3 easy steps

  1. Notification

Notify your supervisor and HR at least 30 days in advance of your need for medical leave. If your need for medical leave is an emergency, notify your supervisor and HR as soon as practicable…this can also be done by a family member if you are incapacitated. 

  1. Application

Have your healthcare provider (or your family member’s healthcare provider)  complete a Medical Certification Form and fax it to (360)788-0071 or email it to HR.Disability@wwu.edu.

  1. Leave Plan Review
